What standards are used to determine if a sidewalk needs repair?

The standards are:
1) Formation of a lip - 3/4" trip hazard on any surface.
2) Single crack - any crack 1/2 " or more in width.
3) Multiple cracks - 3 or more cracks in any one flag.
4) Rough surface - any hole 1" deep or spalling with exposed aggregate.
5) Ponding water - any ponding that creates a nuisance.
