You can obtain a death certificate for someone who died in the City of Warren by providing the following: Name of deceased and date of death. Certified copies are $7.50 for the first copy and $3.00 or each additional copy requested at the same time. A check or money order payable to the City of Warren is required. The request forms are available on the City Clerk's website here.
