The person named on the birth certificate or either parent named on the birth certificate can request certified copies of a birth certificate if the person was born in the City of Warren. The following information is required: name of person, date of birth, mother's maiden name, father's name, driver's license number of person requesting certificate and signature. Certified copies are $7.50 for the first copy and $3.00 for each additional requested at the same time. A check or money order payable to the City of Warren is required.
